SAN JOSE — A health care titan is eyeing major new medical facilities and a big revamp of an existing hospital complex in San Jose, according to plans that have been submitted to city officials.

Good Samaritan Hospital, which operates a big medical center in southwest San Jose, has proposed wide-ranging changes at its campus on Samaritan Drive, a very preliminary proposal on file with the city shows.
